Heavy items like books and CDs should be put in a smaller box so they would be easier to lift. This would also ensure that the box would not get torn while being loaded and unloaded. You can also use a bigger box. You should put fill half of the box with lighter items such as comforters and the like.
If you have to transport desks and other furniture, they would be easier to transport when dismantled. You should also pack similar items together. One way to ensure this is to pack by rooms.
To avoid damage, make sure you provide enough cushion in the box. You should also fill empty spaces inside too. Then in the truck or van, you should also make sure that the boxes would not shift. You can use load straps to ensure this.
